    助力垂体腺瘤评估识别 中国团队研发出深度学习模型

    　　中新网北京12月19日电 (记者 孙自法)中国科学院自动化研究所12月19日向媒体通报说，该所与多家医院组成的联合团队，通过头部磁共振成像(MRI)结合弱监督学习方法，研发出垂体腺瘤进袭性行为评估的深度学习模型，可无创进行垂体腺瘤进袭性行为的术前评估，为垂体腺瘤精准治疗提供重要指导依据。 　　这项可帮医生提前识别垂体腺瘤中的“难缠者”、助力精准个体化治疗的重要研发进展，由中国科学院自动化研究所、首都医科大学附属北京天坛医院与郑州大学第一附属医院、首都医科大学附属北京潞河医院等合作完成，相关成果论文近日在施普林格·自然旗下专业学术期刊《NPJ数字医学》发表。 垂体腺瘤侵袭性生长。中国科学院自动化研究所 供图 　　研发团队介绍说，垂体腺瘤又称垂体神经内分泌肿瘤，约占所有颅内肿瘤的10%-20%，据报道其患病率为每10万人中70-100例。尽管垂体腺瘤被归类为良性肿瘤，但超过40%的病例在病程进展中会表现出肿瘤侵袭性，难以通过手术完全切除，进而导致残留肿瘤快速复发。20世纪提出的两种分级通过评估肿瘤侵犯的解剖区域(侧方或上下方)来判断垂体腺瘤的侵袭性，主要以影像学检查和手术中的观察确定。该方法虽操作简便，但难以关联垂体腺瘤侵袭包绕周围正常组织、类似恶性肿瘤的生物学行为，因此对临床诊疗的指导价值有限。 　　2017年前后，世界卫生组织与欧洲内分泌学会提出“进袭性”垂体腺瘤的概念，更强调肿瘤的生物学行为。进袭性肿瘤的核心特征包括侵袭性生长、快速增大、易复发、病理标志物异常及治疗抵抗等。由于进袭性概念涵盖维度广泛，且目前缺乏公认的标准化评估工具，直接精准评估垂体腺瘤的进袭性水平面临挑战。当前多数相关研究难以实现对肿瘤进袭性的全面多维度评估，这导致临床中难以精准识别具有高进袭性行为的垂体腺瘤并及时进行临床关注，也难以制定更全面的个体化诊疗方案。 本项研究相关示意图。中国科学院自动化研究所 供图 　　在本项研究中，中国团队首次提出基于侵袭特征的弱监督标签深度学习模型，结合磁共振成像对垂体腺瘤进袭性进行评估与识别，并通过多维度、多任务验证系统考察了其效能。该模型在侵袭标签的弱监督指导下，利用深度学习网络提取特征、筛选特征后，最终为每位患者生成个体化评分。 　　结果显示，模型评分与进袭性呈现密切的正相关关联，不仅能显著反映肿瘤侵袭状态，还可辅助评估肿瘤复发等不良预后、病理标志物异常及恶性肿瘤相关生物学通路的情况。目前，这一结果已在全国多家医院多中心队列中得到验证，充分证实模型在复杂、多变临床环境下的稳定性与可靠性，为临床广泛推广应用奠定坚实基础。 　　同时，本项研究已将复杂的多步骤磁共振成像图像处理及相关算法整合至在线平台，临床医生或相关使用者仅需上传磁共振成像数据，即可快速获取垂体腺瘤进袭性的智能评估结果，大大降低了人工智能技术的应用门槛。 　　研发团队透露，下一步将立足临床诊疗实际需求，持续推进模型与平台的迭代优化，加速科研成果从实验室走向临床一线，助力提升垂体腺瘤临床诊治的精准化水平，切实惠及广大患者。(完) 【编辑:张令旗】|dfg|
