    千万不能这样吃鱼！伤肝又坏胆！

    　　又到了冬令进补的时候，短视频平台上，一盘盘“晶莹剔透”的生鱼片，配上“鲜甜脆嫩，一口入魂”的解说词，刺激着人们追求新奇美食的欲望。但很少有人知道，这份“鲜美”背后，可能潜伏着一个致命威胁——肝吸虫(学名“华支睾吸虫”)。今天就带大家看清肝吸虫的真面目。 　　一、肝吸虫传播的“三步曲” 　　肝吸虫的传播离不开“三大宿主+一种习惯”，即第一、第二中间宿主和终宿主，还有生食淡水鱼的习惯。其完整的生命周期严格遵循“第一中间宿主(淡水螺)→第二中间宿主(淡水鱼)→终宿主(人/哺乳动物)”的顺序；而生食淡水鱼这一习惯起到了关键作用，帮助肝吸虫顺利从第二中间宿主进入终宿主。 　　第一步：在第一中间宿主体内孵化 　　肝吸虫虫卵经粪便排出体外，在水中被淡水螺吞食，在螺体内开始孵化，开启无性增殖。 　　第二步：在第二中间宿主体内结囊 　　每颗虫卵在螺体内经过无性增殖后，都会变成无数个尾蚴从螺体内逸出，尾蚴在水中遇到淡水鱼便迅速钻入鱼体内，每条尾蚴都能在肌肉里发育成一颗囊蚴。 　　第三步：通过摄食侵入终宿主 　　待囊蚴在鱼体内发育成熟后，便迎来了最关键的一步：侵入终宿主。但这一步光靠它自己是完成不了的，必须有“贵人”相助。喜食生鱼片的人便充当了这个关键角色，为肝吸虫完成生活史，壮大种群数量做出了巨大“贡献”。 　　表面看上去晶莹剔透的生鱼片，在显微镜下并不一定干净，含有囊蚴的生鱼片进入人体消化道后，囊蚴在消化液的刺激下脱囊成为幼虫，幼虫沿肠道钻入肝胆管并在此定居，经过一个月发育为成虫开始产卵，肝吸虫一个完整的生活史就这样完成了。 　　二、鲜味下暗藏的“渐进式”伤害 　　感染初期1-2个月，部分人会出现腹痛、腹泻、乏力等胃肠道症状，若感染的虫不多也可能没有明显症状。随着成虫在胆管内长期寄生，会持续刺激胆管壁，导致胆管壁增生、胆管狭窄、胆汁淤积，进而引发慢性胆囊炎、胆结石。 　　肝吸虫成虫可在人体内存活20-30年，长期寄生会对肝脏和胆道系统造成持续性损伤，危害程度随感染时间和虫体数量递增，最终会诱发胆管癌。在2009年，世界卫生组织下属机构国际癌症研究署将华支睾吸虫列为明确的致癌物。 　　面对肝吸虫这种累积式的伤害，我们既不要心存侥幸，也不必过度担心，去疾控机构或者医院的专科门诊做个粪便虫卵检测，就可以明确诊断。若感染了肝吸虫，在医生的指导下服用驱虫药即可药到虫除。 　　三、科学防治，守护“肝”净 　　除了人之外，在自然界还有其他吃鱼的各类哺乳动物，比如猫、狗等可以作为肝吸虫的终宿主，所以肝吸虫在自然界的分布也很广泛。预防肝吸虫的目的是保障人们的健康，所以与其把肝吸虫及其宿主从自然界中消灭，不如阻断肝吸虫侵入人体的这一环节。 　　最关键的措施就是管住嘴，拒绝“生食诱惑”，肝吸虫不耐高温，只要煮熟就可以杀灭鱼肉中的囊蚴，做到真正的干净。同时提倡生熟厨具分开，让风险远离餐桌；其次是规范水产养殖业，从源头减少养殖鱼类的感染；最后是做好人畜粪便管理，防止虫卵污染水源。吃干净的鱼，做“肝”净的人，远离肝吸虫威胁！/健康科普 　　作者：寄生虫病所 诸廷俊 　　审核：李石柱 　　编辑：杨紫萱、张林林 　　监制：姚建义、于子涵 　　(“中国疾控中心”微信公众号) 【编辑:张令旗】|dfg|
